Job Description

The Behavioral Health Case Manager oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Being a Behavioral Health Case Manager typically reports to the head of a unit/department. Acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. In addition, Behavioral Health Case Manager may require a master's degree of social work (MSW) and a state license to practice clinical social work or a Professional Counselor license.
